No passado dia 10 de Maio de 2023, a Direcção Geral de Saúde (DGS) publicou a Orientação 02/2023 | Cuidados de saúde durante o trabalho de parto, para Profissionais de Saúde do Serviço Nacional de Saúde de hospitais com Serviços de Obstetrícia e Ginecologia, de Pediatria/Neonatologia e de Anestesiologia.
O Observatório de Violência Obstétrica em Portugal (OVO PT) felicita a iniciativa de se criar uma orientação relativa aos cuidados durante o trabalho de parto e felicita, particularmente, o facto de ter sido auditado o Colégio da Especialidade de Enfermagem de Saúde Materna e Obstétrica (CEESMO) e a Associação Portuguesa de Enfermeiros Obstetras (APEO). Foi com agrado que o OVO PT viu reconhecida autonomia aos/às Enfermeiros/as Especialistas de Saúde Materna e Obstétrica, embora esta autonomia não esteja totalmente em conformidade com o Regulamento de Competências Específicas dos EESMOs (Regulamento n.º 391/2019 – Diário da República n.º 85/2019, Série II de 2019-05-03) e do parecer da Mesa do Colégio da Especialidade de Enfermagem de Saúde Materno Obstétrica nº 22/2023 (1) de 03 de Maio de 2023, intitulado “Assistência Pré-natal das Grávidas de baixo risco”.
Ao mesmo tempo, o OVO PT salienta que esta orientação da DGS, contempla somente o Serviço Nacional de Saúde (SNS), não estando os Sistemas de Saúde/Hospitais Privados abrangidos pela mesma, aumentando o fosso entre a saúde Pública e a Privada.
Desde 2022 que é clara a instabilidade vivida no SNS, com urgências obstétricas e blocos de parto fechados e em regime de alternância, enquanto o sistema privado ganha com esta fragilidade. Perante esta situação há Mulheres a fazer mais de 100 km para parir, o que aumenta os casos em que os partos acontecem acidentalmente e sem assistência fora dos hospitais (2), ou nas ambulâncias – principalmente em zonas com menos hospitais disponíveis como Algarve ou Guarda – pondo em risco as vidas destas mulheres e bebés.
Em simultâneo, o CEESMO defende a autonomia, que resultaria na libertação de médicos obstetras, e deste modo, reorganizando os profissionais de saúde, permitiria que todas as urgências do país estivessem abertas. É claro que este regulamento não foi contemplado nesta Orientação da DGS, onde a autonomia do EESMO não é total, uma vez que quaisquer decisões/acções remetem sempre ao Médico Obstetra de serviço.
Paralelamente, desde o início da formação do OVO PT que tem havido tentativa de contacto com a OM, o que nunca se concretizou. Parece, portanto, que a OM, ao contrário da OE, não prioriza a voz das mulheres nem os seus interesses. O OVO PT lamenta mais uma vez, que não tenham sido convidados ou integrasse representantes da população, tais como: associações que trabalham questões relacionadas com a vida sexual e reprodutiva das mulheres, associações de estudantes na área da saúde, sindicatos de medicina e enfermagem, movimentos cívicos pelos direitos das mulheres e organizações não governamentais.
Concomitantemente, vem o OVO PT salientar que, pela bibliografia consultada no documento, é possível verificar que a presente orientação não se baseia na melhor e mais recente evidência científica nem atende a uma “ampla revisão da literatura” sobre o tema. A menção a publicações com mais de 30 anos, pouco robustas, desiludem tremendamente, mostrando o pouco interesse em seguir as mais actuais evidências. O OVO PT destaca, assim, os aspectos negativos da orientação, por não se basearem em evidências científicas, por serem contrários às Recomendações da Organização Mundial da Saúde (OMS) e por serem igualmente contrários à Lei n.º 15/2014, de 21 de Março (3) em alguns pontos, que se concretizam de seguida:
1.Realização de episiotomia, quer seja para abreviar o nascimento ou quando a laceração perineal está iminente ou a iniciar-se.

ROMS: 39. Routine or liberal use of episiotomy is not recommended for women undergoing spontaneous vaginal birth.
- Although the review evidence on comparative effects of episiotomy policies was presented as selective/restrictive versus routine/liberal use of episiotomy, due to the beneficial effects of selective/ restrictive compared with routine/liberal episiotomy policy, the lack of evidence on the effectiveness of episiotomy in general, and the need to discourage the excessive use of routine episiotomy across all settings, the GDG felt that it was important to emphasize that routine/liberal use of episiotomy is “not recommended”, rather than recommending the selective/restrictive use of episiotomy.
- The GDG acknowledged that, at the present time, there is no evidence corroborating the need for any episiotomy in routine care, and an “acceptable” rate of episiotomy is difficult to determine. The role of episiotomy in obstetric emergencies, such as fetal distress requiring instrumental vaginal birth, remains to be established.
- If an episiotomy is performed, effective local anaesthesia and the woman’s informed consent is essential. The preferred technique is a medio-lateral incision, as midline incisions are associated with a higher risk of complex obstetric anal sphincter injury (OASI). A continuous suturing technique is preferred to interrupted suturing (168).
- Episiotomies do not warrant the routine use of prophylactic antibiotics, as general infection control measures should be respected at all times (114).
2. Toques vaginais de 4 em 4 horas na fase latente, de 2 em 2 horas na fase activa e de 30 em 30 min no período expulsivo, existindo orientações para a sua diminuição pela OMS.
ROMS: 16. Digital vaginal examination at intervals of four hours is recommended for routine assessment of active first stage of labour in low-risk women.
- This recommendation has been integrated from the WHO recommendations for prevention and treatment of maternal peripartum infections (114), in which the GDG for that guideline determined it to be a strong recommendation based on very low-quality evidence.
- There is currently no direct evidence on the most appropriate frequency of vaginal examinations to prevent infectious morbidity in the mother and baby, and therefore this recommendation was based on consensus reached by the GDG, and it is in agreement with a similar recommendation in the 2014 WHO recommendations for augmentation of labour (46).
- Priority must be given to restricting the frequency and total number of vaginal examinations. This is particularly crucial in situations when there are other risk factors for infection (e.g. prolonged rupture of amniotic membranes and long duration of labour).
- The GDG acknowledged that the frequency of vaginal examinations is dependent on the context of care and the progress of labour. The group agreed that vaginal examinations at intervals more frequent than specified in this recommendation may be warranted by the condition of the mother or the baby.
- Vaginal examinations of the same woman by multiple caregivers around the same time or at different time points should be avoided. The group noted that this practice is common in teaching settings where multiple cadres of staff (or students) perform vaginal examinations for learning purposes.

3. Desinfectar a vulva, realizar massagem e protecção perineal
ROMS: 26. Routine vaginal cleansing with chlorhexidine during labour for the purpose of preventing infectious morbidities is not recommended.
- This recommendation has been integrated from the WHO recommendations for prevention and treatment of maternal peripartum infections (114), in which the GDG for that guideline determined it to be a strong recommendation based on moderate-quality evidence.
- This recommendation was based on the lack of clinical benefits for the neonate and not on the potential effect of the intervention on group B Streptococcus (GBS)-related maternal infectious morbidity.
- The GDG acknowledged the considerable variations in policies regarding the screening for GBS colonization in pregnant women. Therefore, the group agreed that this recommendation should be implemented within the context of local policy and guidance on screening for GBS colonization.

ROMS: 38. For women in the second stage of labour, techniques to reduce perineal trauma and facilitate spontaneous birth (including perineal massage, warm compresses and a “hands on” guarding of the perineum) are recommended, based on a woman’s preferences and available options.
- Evidence suggests that perineal massage may increase the chance of the keeping the perineum intact and reduces the risk of serious perineal tears, that warm perineal compresses reduce third-and fourth-degree perineal tears, and that a “hands-on” approach (guarding) probably reduces first-degree perineal tears. Most women accept these low-cost preventative perineal techniques and highly value the outcomes that they impact.
- Evidence on Ritgen’s manoeuvre (using one hand to pull the fetal chin from between the maternal anus and the coccyx, and the other hand placed on the fetal occiput to control speed of birth) is very uncertain; therefore, this technique is not recommended.

4. Uso de CTG contínuo em trabalhos de parto espontâneos.
ROMS: 17. Continuous cardiotocography is not recommended for assessment of fetal well-being in healthy pregnant women undergoing spontaneous labour
The evidence on beneficial effects of continuous CTG suggests that it might not be cost-effective when compared with IA since the only clear benefit is a small absolute reduction in neonatal seizures (with a rate of 1 fewer per 1000), and the longterm effects are unclear. Given that the use of CTG could also lead to moderate increases in costly labour interventions, such as caesarean section, instrumental vaginal birth and fetal sampling, which are associated with additional risks of morbidities for the mother and the baby, avoiding its use could lead to substantial cost savings. Health care costs related to procuring CTG equipment for the labour ward, with the associated maintenance costs, or use of ancillary resources such as pH monitoring, can instead be used to ensure access to other basic facilities.
Evidence from the same qualitative review suggests that women might place a higher value on avoiding the risk of additional interventions (e.g. caesarean section, instrumental birth and fetal blood sampling) that make birth abnormal and which override their control of the birth process, without necessarily improving outcomes for them and their babies (23). In addition, continuous CTG during labour could negatively impact on a woman’s sense of autonomy during the birth process, by increasing her discomfort and reducing her choices with regard to mobility and pain relief options.
5. Controlo da ingestão de líquidos e restrição da ingestão de sólidos durante o trabalho de parto.
ROMS: 24. For women at low risk, oral fluid and food intake during labour is recommended.
- This recommendation has been integrated from the WHO recommendations for augmentation of labour (46), in which the GDG for that guideline determined it to be a conditional recommendation based on very low-quality evidence.
- Given that restriction of oral fluid and food intake has no beneficial effects on important clinical outcomes, including the use of labour augmentation, the GDG puts its emphasis on respect for the wishes of the woman and therefore made a positive recommendation.
- The GDG noted that no cases of Mendelson’s Syndrome (inhalation of food and drink from the stomach into the lungs during general anaesthesia – the most important safety concern limiting oral intake during labour – were reported in over 3000 women participating in the trials included in the systematic review.

As recomendações da OMS contemplam vários estudos actuais e atendem as necessidades reais das grávidas em trabalho de parto, colocando-as no centro das suas decisões. Além disso, O OVO PT sublinha que a recomendação de realização de massagem no períneo e o elevado número de toques vaginais, exposição e escrutínio desnecessários, podem ser vividos também como uma violência sexual, no contexto de violência obstétrica.
Considera o OVO PT que esta orientação apresenta um retrocesso relativamente ao reconhecimento da autonomia das mulheres, que não teve em consideração dados epidemiológicos, tais como os anunciados no estudo (4) desenvolvido no âmbito do projeto IMAGINE EURO, publicado na revista “The Lancet Regional Health – Europe” .
Recomenda o OVO PT que a orientação 002/2023 seja revista, com o objectivo de ir ao encontro das recomendações da OMS, transitando para um modelo biopsicossocial, em total respeito pela autonomia das pessoas grávidas, parturientes, puérperas, fetos e recém-nascidos.
Ainda que tal não suceda, o OVO PT reforça, perante a comunidade, a prevalência da Lei e dos Direitos Humanos sobre qualquer orientação ou protocolo, nomeadamente a prevalência do direito ao consentimento informado. Ou seja, nenhum protocolo hospitalar ou recomendação, se pode sobrepor à legislação em vigor.
O OVO PT, enquanto associação multiprofissional que surge da necessidade de observar e denunciar publicamente a incidência das práticas que constituem violência obstétrica, vem denunciar publicamente a orientação 002/2023 como mais um instrumento ao serviço da violência obstétrica, através do qual se tentou lograr uma mínima mudança para escamotear o ambiente de violência obstétrica vivido nos hospitais portugueses, tanto no sector público como no sector privado.
Mantemos, mais uma vez, total disponibilidade para o diálogo com qualquer organismo público, para que consigamos ter total transparência na realidade da saúde materna em Portugal.
